Job Description
A bit about this role:
- We are looking for a forward-thinking, innovative, clinically excellent cardiologist to deliver high quality, top-notch cardiovascular care to Devoted Health plan members and to serve as the supervising physician for the Devoted Medical Heart Center nurse practitioner team.
- Devoted Medical is building a next generation value based, virtual-first cardiovascular care delivery model called the Devoted Medical Heart Center. The Heart Center encompasses our virtual general cardiology clinic, our hypertension disease management program, our congestive heart failure disease management program, and our cardiometabolic clinic.
- You will serve as the inaugural Cardiology Attending with a primary focus on delivering direct virtual-first general cardiology care that is superior to general cardiology care in the community.
- You will also work closely with our Congestive Heart Failure, Hypertension and Cardiometabolic clinical programs to offer clinical supervision as needed. Each of these programs are highly successful “micro centers of excellence” that deliver highly tailored, specialized care to patients with cardiovascular chronic disease and preventive care needs. These programs are led by interdisciplinary teams of nurse practitioners, nurses, health coaches and pharmacists.
- All of our clinical programs are supported by remote weight and blood pressure monitoring, extended Holter capabilities, and in-home diagnostics (including echocardiogram, vascular ultrasound, and EKG).
Required skills and experience:
- You believe strongly in an innovative, value-based approach to cardiovascular care focused on clinical quality, patient experience, and improved access to cardiology services.
- Board certified in Internal Medicine and Board Certified in Adult Cardiovascular Disease
- Proficiency in EKGs, Holter monitoring, and echocardiography.
- You must be comfortable delivering virtual cardiology care for clinically eligible cardiac conditions through telehealth appointments, remote monitoring, in-home diagnostics, and collaboration with community cardiologists when clinically indicated.
- Strong communication skills especially with community cardiologists and PCPs as well as internally with a cross-functional team at Devoted Medical including clinical (nurses, medical assistants, nurse practitioners, other physicians), operations, and product partners.
- Agile mindset open to a high impact, novel care model which may undergo rapid iterations to achieve continued improvements in clinical and quality outcomes.
Desired skills and experience:
- Fluency in Spanish is a plus though not required.
- Prior experience practicing in value based care models.
- 3-5 years post Adult Cardiology Fellowship training.
Your Responsibilities and Impact will include:
- This is a full-time role.
- You will provide best-in-class direct telehealth care in our virtual cardiology clinic. The breakdown of your time may approximate:
- 75% direct telehealth cardiology care.
- 15% nurse practitioner collaboration.
- 10% project based work to help continue to grow the cardiovascular program.
- Expect ample time for visits in this value-based model with initial visits lasting 45-60 minutes.
- There are no in-person clinic based or hospital based responsibilities.
- Urgent calls after hours will be triaged to our 24/7 care on demand line. There are no overnight call responsibilities. You will be asked to help serve as a backup to the nurse practitioners on call for our care on demand (urgent care) clinic for approximately one weekend a quarter.
- While our clinic is virtual, you will partner with our care coordination team to help members access high-quality community cardiology care when in-person care is required or preferred.
- As the clinic grows over time you will help build a collaborative model with cardiology nurse practitioners helping to support patients’ general cardiology needs.
- While you will be focused on the delivery of cardiology care, you will be a core part of helping iterate the clinical model to achieve better outcomes for our members in line with the Devoted mission of treating every member like family.
